Existing customers can register for telephone banking by printing and completing the attached forms:- 1. Telephone Banking form 2. Details of Signatories form (where appropriate) - this form should only be completed by any signatory who is not going to become a telephone banking user and who has not previously been required to prove their identity to Unity Trust Bank. Please circulate the 'Terms and Conditions 2008' booklet and 'Information on proving your identity' document to each person who applies to become an authorised user. 3. Change of Signatory Request form - this form is to notify us of changes to signatories on your account. You can notify us of a change by printing and completing this form. Please ensure that all new signatories read the three documents below ('Terms and Conditions 2008', 'Information on proving your identity' and 'Customer Information & how we use it') before completing the form. Once completed, please return the form to FREEPOST, Unity Trust Bank, Nine Brindleyplace, Birmingham B1 2BR. You do not need to use a stamp.
